Clinical Supervision Training Program Service Agreement
By registering for the Clinical Supervision Training Program through the Center for Ethical Social Work Practice (CESWP), I understand and agree to the following:
Registration, Payment, and Training Sequence
My registration is for the training level(s) and dates I select during checkout. The Clinical Supervision Training Program is designed to be completed sequentially: Level One must be completed before Level Two, and Level Two must be completed before Level Three.
When selecting my training dates, I am responsible for choosing dates that maintain this sequence. I may select dates from different cohorts, provided that I complete the levels in order. For example, I may complete Level One in March, Level Two in July, and Level Three in November, but I may not attend Level Two or Level Three before completing the preceding level.
I am responsible for reviewing my selected training dates for accuracy and appropriate sequencing before completing registration.
Refunds and Cancellations
Registration fees are non-refundable. Because enrollment reserves space in the program and CESWP incurs costs associated with planning and delivering the training, refunds are not provided if I later choose not to attend or am unable to participate.
CESWP may, at its sole discretion, make exceptions to this policy in extraordinary circumstances. Any exception is considered individually and does not establish an ongoing policy or guarantee that similar circumstances will result in a refund.
If CESWP cancels a training and cannot provide a reasonable alternative, participants will be offered an appropriate remedy, which may include transfer to another training date or a refund of the affected registration fee.
Transfers and Changes to Training Dates
Registration includes one complimentary transfer to another available training date. A second transfer is subject to a $100 transfer fee per level. Transfer requests must be submitted at least 14 days before the scheduled training date and are subject to availability.
Participants purchasing a multi-level package must complete the package within 12 months. Changing training dates does not extend the 12-month completion period unless CESWP expressly approves an exception.
Continuing Education and Credentialing
I understand that I am responsible for determining whether this training meets the licensing, credentialing, continuing education, or employment requirements applicable to me in my jurisdiction or professional setting.
I understand that I must meet CESWP's applicable attendance, participation, evaluation, and other completion requirements in order to receive continuing education documentation or a certificate of completion.
Email Communications
I understand that CESWP will use the email address provided during registration to send communications necessary to administer and deliver my training, including registration information, reminders, training materials, evaluations, scheduling information, and other program-related communications.
By registering, I also agree to be added to the Center for Ethical Social Work Practice email list and may receive future educational content, professional resources, training announcements, and other CESWP communications. I may unsubscribe from marketing emails at any time.
Unsubscribing from marketing communications does not prevent CESWP from sending transactional or service-related communications necessary to administer or fulfill my registration.
Participant Responsibility
I am responsible for providing accurate registration information, monitoring the email address used to register, attending the training dates I select, and completing applicable requirements for participation and continuing education documentation.
By completing my purchase, I acknowledge that I have reviewed, understand, and agree to this Service Agreement.
